About us 

Recovery in Community is a collaborative initiative by Manchester Mind and Gaddum, two leading mental health charities in Manchester.

We support people to leave mental health inpatient care and stay well in the community.

We say that ‘Recovery’ means:

Movement towards thriving from a difficult point in life – through developing new ways to deal with things and manage the ups and downs of life.

We believe: 

  • There is no benchmark for what a fulfilling life looks like.
  • While not linear or the same for everyone: recovery is possible and is unique to each individual.
  • Everyone has the right to make choices about how to live their life alongside their mental health.
  • It is important to work with people where they are at and value the self-knowledge they bring from what they have lived through.
  • There is much to learn for all of us about mental health and recovery.
  • Community, connection and social support are essential in recovery.
  • Collaboration with partners is the key for success as we work with each person towards better outcomes.

 

Our Goals

We work alongside people to help them move forward in their recovery journey, from hospital discharge and beyond.

 

  • Get people home: when people are sent to an out of area placement (OAPs), we take action to get them back to Greater Manchester so they can recover closer to their community.  
  • Discharge: We support people when they’re ready to be discharged from inpatient mental health services to get back to the community.  
  • Recovery: We work with people, post-discharge, to ensure they continue to get the support they need to recover, make informed choices, stay well and ultimately thrive.

Our Team

We’re a multi-disciplinary team, this means we have lots of different roles on the team so we can meet the needs of different people who need our service.

Our team includes: 

  • Case worker: expert, experienced case workers who take action and practical steps to support discharge planning and community support post discharge. 
  • Young Person’s Caseworker: our Children and Young People’s specialist supports people under 25 get access to the resource they need 
  • Peer support worker: people with lived experience of inpatient mental health care who use their lived experience as a strength to support people. 
  • Housing & Welfare Right Advisor: provides advice and support to solve housing issues and ensure people have access to income via welfare benefits. 

 

What we do

We know mental health is very personal so we provide support tailored to the individual based on what they need to leave hospital and stay well.

We take a strengths based, trauma informed approach to coproduce a plan with the people we supportWe meet people when they’re in hospital, build a relationship and support them for 6 months post discharge. 

 

Examples of support we provide includes: 

  • Care and support planning including practical steps 
  • Access to community services, GPs, education or training 
  • Wellbeing checkins 
  • Peer Support 
  • Support for the person’s carer 
  • Culturally informed care and support 

 

How to refer

We currently take referrals from people who are inpatients in Manchester mental health beds and Manchester residents in out of area placements (OAPs) or Independent Sector beds
