Palliative care for families

Who is this service for?
We provide therapeutic support for families of children and young people with complex or life limiting conditions.

Who can access it?
This service is available in the following areas: Salford, Stockport and Rochdale.

How do I access this service?
Firstly speak to your NHS Community Children’s Nurse who will submit a referral for you. Once that’s done, one of our team will contact you to set up a first meeting where we’ll assess what it is you need from our service.

We’re here to offer support to parents and families who have a child with a life limiting or complex illness.

We work in partnership with the NHS Community Children’s Nursing teams in each of the areas we operate to ensure you get the best level of care.

The type of support that we offer is sometimes referred to as ‘person-centred’. This simply means that you’ll be the focus of the sessions and you decide what it is that you need to talk about. Sometimes this might just be a chat about what’s happening for you and other times it might be deeper. It’s your choice.

We understand how busy life is as a parent or family of a child with a complex or life limiting condition, so we’ll discuss with you when and where are the best times to see you. More often than not we come to your home as it can be the easiest option for you.
